TBDHU launches online consent for school vaccine clinics

TBDHU has launched a new secure online form to make providing consent for school-based vaccines easier and more convenient for parents and guardians across Thunder Bay District. The new online form takes minutes to complete, can be viewed on a mobile phone, and is available in English and French.

“The online consent form was developed in response to feedback from parents and school staff and we are excited to be able to offer this option,” said Laura Dennis, TBDHU’s Manager of Vaccine Preventable Disease. “We want to make providing consent easy for busy parents and guardians.”

Parents and guardians are encouraged to provide feedback on the online form so that TBDHU can continue to improve the process. A survey to gather feedback on the form will be available on TBDHU’s website beginning September 23.

Timelines for when parent and guardians across Thunder Bay District can expect to receive their information packages including how to provide their consent online for school based vaccine clinics:

  • Students attending Grade 7 in Thunder Bay area schools have already received their information packages through their schools.
  • Students attending Grade 7 in District communities will also receive their information packages through their schools this fall.
  • Students attending high schools in Thunder Bay and in District communities will receive their information packages from TBDHU by mail in the coming months.

Parents and guardians who have not received an information package can find all documents, including the online consent form, and a school clinic schedule at TBDHU’s School Immunization page at TBDHU.com/schoolimmunizations.
