Grade 7 and 8 Students - School-based Clinics

Second dose vaccine clinics are now underway in Thunder Bay and surrounding rural communities

Students who are currently suspended from school in Thunder Bay can walk-in to get a missing vaccine OR drop off an exemption form:

  • TBDHU clinic at 999 Balmoral; Monday to Friday, 9-11:30am and 1-3:30pm

Two clinics: TBDHU hosts two school-based vaccine clinics for children in Grade 7 each school year. See the tabs below for the clinic schedule, based on where the school is located (Thunder Bay versus District). 

  • Grade 8 students: Students in Grade 8 who are missing Grade 7 vaccines will be given the opportunity to receive missed doses and more information is listed below.
  • Easy and convenient option: These clinics provide an easy and convenient option for students to be immunized and keep up to date on their vaccines, including the vaccines required under the Immunization of School Pupils Act (ISPA).
  • Consent and schedule: See the tabs below for more information on the recommended vaccines. The consent process and a schedule of school clinics is outlined in the sections below based on where the student attends school.

 

Vaccines offered

The following vaccines are offered and recommended

  • *Meningococcal conjugate (Men-C-ACYW) vaccine (1 dose needed); eligible until the vaccine is received.
  • Human papillomavirus (HPV-9) vaccine (2 doses); eligible until August 31 of their Grade 12 year; those who start the series at age 15 and older require 3 doses to complete.
  • Hepatitis B vaccine (2 doses needed); students are eligible until August 31 of their Grade 12 year; those who start the series at 16 and older require 3 doses to complete.

*Meningococcal conjugate (Men-C-ACYW) is required for school attendance in Ontario or a valid exemption must on file with TBDHU. For more information, see the Immunization of School Pupils Act (ISPA) web page.

Consent Process
  • Package received at school: Grade 7 and 8 students will receive a consent form package at their school during the first week of school to take home. Each grade will receive a different package. 
  • Review and submit online by September 12: The consent form is only available online. Those who cannot complete the online version can give verbal consent over the phone by calling 807-625-5908. Submitting before the deadline gives TBDHU staff time to prepare for the school clinic.
  • Missed the return date? Parents/guardians can still complete the consent online up to two (2) days before the clinic OR give verbal consent over the phone until the day before the clinic by calling 807-625-5908. Find the dates of each clinic in the schedule section below.
Students in Grade 8 in September 2025 who are missing doses
  • Available to receive in Grade 8: Students in Grade 8 who  are missing doses can receive them in Grade 8.
  • Consent submitted in Grade 7 still valid: Consents submitted in Grade 7 are valid until the end of Grade 8.
  • No signed consent?  If a consent was not submitted in Grade 7 one can be completed in Grade 8. 
Missed Clinics
  • Notices mailed home after first (1st) visit: All students who miss the first clinic at their school will receive a notice mailed to their home that will list the options available to catch up.
  • Notices mailed home after second (2nd) visit: Only students with a signed consent form will receive a notice in the mail if they miss the second clinic at their school.
  • Options to catch up: The options include receiving at the second clinic, booking an appointment with their health care provider OR taking advantage of catch up Grade 7/8 clinics offered by TBDHU. These Grade 7/8 catch up clinics will require online booking and information on the dates will be included in the notice mailed home. 
Clinic Schedule
  • Grade 7 schools: TBDHU visits all schools with Grade 7 students.
  • Students in Grade 8: Students in Grade 8 can receive missing doses at these clinic; see the section above for more information on the consent process so Grade 8 students needing vaccines can catch up.
  • Second dose clinics: The second visit will be held approximately 6 months following the first dose clinic and will be added below once confirmed.
  • Dates can change: The dates below are subject change and the second clinic dates will be added once confirmed. Please check back before the scheduled dates for your child's clinic to confirm.
